Photography and Videography Consent
By signing this form, you are providing consent for Eski (the trading name of Eskimosoup Ltd, company registration number at 08125529), our clients and selected media partners to feature your image, footage, quotes and/or experience as part of promotional campaign materials. It may take the form of printed materials, press releases, social media content, digital materials, outdoor media, mainstream media and any other media we may use in the future. It will be used for a minimum of two years and will remain on Eski and campaign platforms beyond this time as part of older documents or online posts.
You may be recognised by friends and family although we will aim to remove or hide tags that link to your personal profile online from the page in an effort to protect you. However, we cannot guarantee we will be able to manage every interaction on posts. If you are uncomfortable with this and do not wish to have your image used, please opt out of this shoot.
This form is to confirm that you have received this brief and been given the chance to opt out. Although you can withdraw your consent at any time, please bear in mind that removing any footage from a video shoot proves incredibly difficult for us so if there are any issues, concerns or questions please raise them with a member of the Eski team before you sign this form.
By signing this form, you give permission for the data entered within this form to be stored electronically and securely by Eski for up to 24 months before it is removed. This data will not be shared with any third parties and will be used only for the purpose of confirming consent.
You can request us to stop using your experience or image, or update any details, at any time by emailing mail@eski.media however, please be aware that once a piece of media has been posted online it is publicly available.
